- Step 1
  Complete the Marriage Registration Form.

  Contact Fr. Raju or Ileana at (813) 949-5154 to arrange an Initial Meeting.
- Step 3
  
  Pre-Marital Inventory (PMI)
  
  A psychological relationship survey required by our Diocese. Must begin at least 6 months prior to the wedding date. Form provided at Initial Meeting.
  
  Complete Witness Forms (B Forms)
- Step 4
  Couples must attend a Marriage Preparation Workshop conducted by Our Lady of the Rosary Catholic Church. Please refer to the Church Bulletin and Parish Calendar for event dates.
- Step 5
  Attend a required online Natural Family Planning Learning Session. Completion certificates must be emailed to [email protected].
- Marriage Donations

- *If the couple is planning to be married at another parish, they will schedule a “Wrap Up” session to prepare their wedding file to be sent to the parish where the couple will be making their vows.
